Bakalářská práce se zabývá problematikou při propuštění pacienta z nemocnice do domácí péče. Práce je rozdělena na část teoretickou a praktickou. V teoretické části jsou vymezeny pojmy ošetřovatelství, komunitní ošetřovatelství, komunitní péče, domácí péče. Zvláštní pozornost je věnována organizaci domácí péče, specifikům práce sestry v domácí péči při uspokojování potřeb klientů a spolupráci sestry s rodinou. Praktická část je zaměřena na zjištění, zda byli klienti při propuštění z nemocnice informováni o možnosti využití služeb domácí péče, jak získává management domácí péče informace o klientovi a zda sestry z nemocnice a domácí péče spolupracují. |

Bachelor thesis deals with theissue of when the patient is discharged from hospital to home care. The work is dividend into theoretical and practical. In the theoretical partit defines the concepts of nursing, community nursing, community care, home care. Particular attention is paid to the organization of home care, the specifics of nursing home care in meeting the needs of clients and cooperation of nurses with the family. The practical part is focused on whether clients were being released from the hospital informed on the use of home care services as home care management obtains informations about the client and whether the nurses from the hospital and home care together. |
